(1) In-Network Insurance: Coverage and fees are determined by your health insurance provider. Please be aware that most insurance providers require a diagnosis to demonstrate “medical necessity” for the coverage of services. Diagnostic evaluation will serve several functions in our work together, including insurance coverage as appropriate; however, the insurance coverage requirements will not, in themselves, compel a diagnosis nor influence the evaluation process. Personal growth, emotional support, skill building, insight development, and a variety of other purposes are valid reasons to participate in counseling and psychotherapy, with or without a diagnosable mental disorder. If your current experiences of mental health do not meet the criteria for diagnosis, options 3, 4, and 5 (below) will be available methods for us to continue working together.
(2) Superbill: Billing invoice, diagnosis and procedure codes, and other necessary documentation will be provided to you to submit to your insurer for reimbursement for services out-of-network. If your insurance will not provide reimbursement, options 4 & 5 will remain available to cover the current and subsequent appointments.
(3) Direct pay- Flat fee: $250 per intake/ $200 per session
(4) Sliding scale- Flat Fee: $70

(A) Income only:
You may choose to represent your annual income as the sole indicator for your fee scale.
(B) Adjusted income:
You may subtract the yearly sum of your payments for student loans, healthcare debts, and/or immigration fees from annual income to determine your fee scale.
(C) Consulted adjustment:
You may request other personal debts or liabilities to be subtracted from annual income by discussing circumstances and plans with me.
(D) Distributive justice plan:
As an act towards social equity, you may include your assets, investments, inheritances, and other sources of wealth to raise your fee scale.
(E) Mutual care plan:
You can decrease your fees to a lower scale by contributing half the difference as a donation to one of the organizations outlined on the last page of this document.
